Informed consent

Physiotherapy informed consent with a digital signature

Your patient reads and signs the physiotherapy consent form on a phone or tablet before getting on the treatment table, or from home through a signing link. You decide the wording, the version for minors includes their guardians and the signed PDF is filed in their record without you lifting a finger. First month free, no card needed.

The usual paperwork

The consent form that never turns up when you need it

A new patient arrives with minutes to spare and the form is left half-filled at reception. The 15-year-old player booked in for a sprained ankle needs her mum's authorisation, and her mum couldn't come today. And before a dry needling session you wonder whether the sheet signed months ago even mentioned invasive techniques.

With informed consent handled digitally, all of that goes away: each patient signs the text you prepared, the signature is recorded with date and time, and the document is saved in their file, next to their physiotherapy clinical record.

How it solves it

Signed, saved and found in seconds

From template to signature to filed PDF, without a printer or scanner.

📝

Wording that is yours

You start from a ready-made physiotherapy text (treatment, possible discomfort, confidentiality and GDPR) and adapt it to the techniques you use, in Spanish, Catalan or English. The patient signs exactly that version.

📲

In the clinic or remotely

Patients sign with a finger on a phone or tablet during the appointment, or get a single-use link by WhatsApp or email to sign from home, with no sign-up anywhere.

🗂️

PDF in the record

Each signature produces a PDF with date and time, stored encrypted among the patient's documents next to their physiotherapy clinical records. There is a version for minors and their guardians.

Step by step

From wording to signature in four steps

1

Review the template

Adjust the ready-made text to the way you work just once, in each language you use.

2

Choose where to sign

Beside the treatment table on your phone or tablet, or with a link the patient opens at home.

3

Patient or guardian signs

No accounts or downloads; for minors their legal representative signs or, if you prefer, both guardians.

4

Filed automatically

The signed PDF shows up encrypted in the patient's record, ready to look up.

Clear responsibilities

What the tool provides and what is still up to you

✅ What My Fisio Agenda does

  • Gives you a ready-made physiotherapy text you can edit.
  • Collects the signature in clinic or remotely, through a link that only works once.
  • Creates the signed PDF with date and time and stores it encrypted in the record.
  • Prepares the version for minors, with guardian 1 and an optional guardian 2.

👤 What falls to you as a physiotherapist

  • Checking that the text describes the techniques you actually use.
  • Explaining the treatment and answering questions before they sign.
  • Collecting a new consent if the plan changes or you add invasive techniques.
  • Keeping clinical documentation for as long as the regulations require.

My Fisio Agenda helps you collect and keep informed consent, but it is not legal advice and does not replace a review of the text by you or an adviser. See how we protect data in security and GDPR.

Frequently asked questions

Questions about informed consent in physiotherapy

Why do I need informed consent in physiotherapy?
Spain's Law 41/2002 on patient autonomy requires you to explain the treatment and obtain the patient's agreement before applying it, in writing for invasive procedures such as dry needling. Their health data must also be handled in line with the GDPR. Having it signed and filed protects both the patient and your clinic.
What does the app's consent text include?
Six sections ready to use: purpose; description of the treatment and possible discomfort (hands-on techniques, exercise, electrotherapy and, where appropriate, dry needling, with warnings such as short-lived pain or stiffness or small bruises after invasive techniques); confidentiality; data protection under the GDPR and Spanish data-protection law; patient rights; and the final statement with the signature. It also asks the patient to mention pregnancy, a pacemaker, medication or allergies. A dedicated clause is added for minors.
Can I adapt the template to my clinic?
Yes. Each physiotherapist edits their own template, section by section, in Spanish, Catalan and English: name the techniques you use, adjust the tone or add whatever you need. If you want the original wording back, you restore it from the same editor.
Can the patient sign from home?
Yes. You create a signing link and send it by WhatsApp or email, or copy it wherever you like. The patient opens it on a phone or computer, reads the full text and signs without registering or downloading an app; each link is good for one signature and expires after 14 days. Before the first visit you can also send the intake form, which collects their details and the signature in one go (that link lasts 48 hours).
How does a patient under 18 sign?
With the version for minors: the document identifies the minor and guardian 1 and, if there is one, guardian 2, who is optional. You can ask one or both to sign, each with their name under the signature, in the clinic or with a separate link for each guardian. If there is no guardian 2, the text adjusts itself.
Where is the signed consent kept?
Signing produces a PDF showing the date and time of each signature, kept among the documents in the patient's record, encrypted, on servers in the European Union with daily backups. No printing, scanning or sorting folders.
Is there a limit on consents per patient?
Each record holds up to three. That lets you keep the initial consent and collect another when the treatment plan changes or when a patient who started as a minor turns 18.
